By registering as a pre-member of AFFMPC ABADAYA Fisherfolks and Farmers Multi-Purpose Cooperative, I agree to: (1) abide by the cooperative's by-laws and policies; (2) attend the required orientation seminars before gaining official membership; (3) provide truthful information and valid supporting documents; (4) make regular savings and share capital contributions as required; (5) understand that loan availment requires two (2) co-makers and, for amounts of ₱10,000 and above, an acceptable collateral document; and (6) accept that submission of false information may be grounds for rejection or termination of membership. This agreement is entered into voluntarily and in good faith.

I authorize AFF-MPC to collect, process, store, and disclose my personal and sensitive personal information submitted in this registration — including my identity, contact, family, and government ID details — in accordance with the Data Privacy Act of 2012 (RA 10173), for the purposes of processing my membership application, credit investigation, and cooperative services, as further described in the Privacy Policy.
